the referred patient as appropriate

an initial consultation with the referred patient to discuss expectations, including benefits and risks
-
4. Acquire
SPRAVATO® based on the patient’s benefit design
-
5. Instruct
the patient to self-administer SPRAVATO® and observe him or her for at least 2 hours
-
6. Share
treatment progress with the referring provider
REMS=Risk Evaluation and Mitigation Strategy.
Coordinating care with the referring HCP
-
Reach out to the referring HCP before the patient’s first SPRAVATO® treatment session.
-
Obtain each patient’s information, including medication history and other information needed for treatment and prior authorization. Helpful resources, such as the Patient Referral for SPRAVATO® Treatment Form, may be found in the Treatment Center Educational Communications Toolkit.
-
Once the patient has started SPRAVATO® treatment, keep an open line of communication with the referring HCP about the patient’s progress. Helpful resources, such as the Patient Treatment Progress Form, may be found in the Treatment Center Educational Communications Toolkit.
